Your first visit gives us the opportunity to learn about your medical history and current foot or ankle concerns, and gives you time to become familiar with your doctor and our office.
What to Bring
- Insurance card
- Current prescription and over-the-counter medication list, including dosage and frequency
- Medical and surgical history
- Recent test results, X-rays or other relevant records
- Completed New Patient forms, when available

Insurance & Payment
Patients are responsible for co-payments and any charges not covered by insurance. Please contact the office with billing questions or to verify whether your insurance carrier is accepted. Questions about individual coverage and benefits should be directed to your employer or insurance company.
